Step 2. Complete a Submittal Form

Using the myLab Portal, you can electronically create your submittal form to ensure each sample is tested for the correct analysis and paired to your account for timely report delivery and invoicing. Watch our myLab Portal Tutorial video for a quick walkthrough of submitting a sample. 


You can also utilize the printable PDFs listed below to fill out your submittal forms. Each sample type has its own submittal form. Be sure to include the following information with all samples you submit:

  • Full Name
  • Address
  • Phone Number
  • Sample ID
  • Desired Analysis
  • Account Number
  • Email address (suggested for rapid return on reports when submitting multiple samples?.)
