Submitting Samples
COMPLETING YOUR SUBMITTAL FORM:
Using the myLab Portal, you can electronically create your submittal form to ensure each sample is tested for the correct analysis and paired to your account for timely report delivery and invoicing. Watch our myLab Portal Tutorial video for a quick walkthrough of submitting a sample.
You can also utilize the printable PDF to fill out your submittal form. You can find a walkthrough of the paper form in our Soil Submittal Form Guide. Be sure to include the following information with all samples you submit:
- Full Name
- Address
- Phone Number
- Sample ID
- Desired Analysis
- Account Number
- Email address (suggested for rapid return on reports when submitting multiple samples.)
SHIPPING YOUR SAMPLES:
Submittal Forms are required in each box of samples. Submittal forms will aid in expediting your sample analysis and help ensure test accuracy for each one of your samples.
A Permit document may also be required when shipping certain sample types.
When using Midwest Labs box, it is very helpful to utilize the space on top to mark each box. Indicating each box as " 1 of 2 " or " 1A of 2A " helps us organize samples as they arrive. Also indicating the sample type helps us identify package without having to open every one.
You have the ability to create shipping labels through the myLab Portal. We have delivery options for UPS, Fedex, and SpeeDee. Please refer to your Shipping Reference Guide for our current label pricing.
